Safety shoes, glasses

Some University of Iowa departments require their employees to wear safety shoes or glasses while working. General Stores facilitates the purchase of these items. The process for this is:

1. Complete a three-part Personal Protective Equipment Requisition and take it with you to the vendor. Call or e-mail Margie Yoder to request a form.
2. The vendor keeps the white and the canary sheets.
3. The vendor returns the white sheet to 130 MBSB with the invoice for billing.
4. Keep the yellow sheet and return it to your department for its records.

Safety shoes
Cost: The University pays the first $90 of the cost of safety shoes. The employee pays the balance plus tax.

Cost: The University pays 100 percent of the cost of approved safety glasses.
