You'll need to complete affadavit, bibliography, and order forms to order UPACS. They are available from us at 208 Iowa House, IMU. If you have any questions, please call our office.

Affadavit
Complete this form if you own the copyright to material that will be included in your UPAC, or if you have already received permission from the copyright owner to reprint the materials.

Bibliography
This form gives us the information we need to secure permission to use copyrighted material. It includes title, author, publisher, ISBN number, page and chapter numbers, and so forth. You can facilitate the process by providing accurate information. And, by providing this information early, we would hope to receive a response from the publisher before the start of classes.

Permission to copy material is granted for one-time use only. A request must be resubmitted each time the material is used.

You'll need to provide your name and other contact information, answer a few questions about the course, and give us production information on this form.
