You and each of you are hereby notified that demand is hereby made of you for the payment of the unpaid rent which was due the date of ________________________, in the total amount of $ ______________. Said rent is due under the terms of rental agreement covering the following premises:
House ( or apartment) known and designated as NO. ___________________________________ Street, situated and located in ____________________________________, Iowa.
Said premises are now used and occupied by you as a(n) __________________.
Said rental agreement provides for the payment of rent by you of $____________ per month on the __________ day of each month.
You are further notified that is my intention to terminate the rental agreement if the rent in the amount of $ __________ is not paid within three (3) days after the service of this notice upon you.
This notice is given to you in accordance with the provisions of Chapter 562A of the Code of Iowa.
