


To verify that your health insurance coverage meets the University’s requirement, bring your actual policy (or a descriptive brochure) and your health policy identification card (ID cards without supporting information are not sufficient documentation) and present them at the University Benefits Office by the first week of classes. Also bring a completed Proof of Coverage Form (see link under "Additional Materials"). Your insurance documents must be in English. If they are not, obtain an official translation before leaving your country. Your insurance documents must clearly indicate the coverage provided for medical costs, and include:
After the University's exemption deadline has passed, only partial exemptions will be granted. You will be required to pay an insurance premium for coverage from the date of the applicable deadline to the date on which you present the exemption documents and they are accepted. Students who are exempted for the entire year must provide documentation of other insurance coverage at the beginning of the next academic year. If you are not exempted for the entire year, you must return to the University Benefits Office every semester you are enrolled in school to be exempted. Failure to do so may result in nonrefundable insurance charges being added to your U-Bill. |
