Iowa Verification Form (IVF)
A form that is required to complete your financial aid application. It is available on ISIS after The University of Iowa receives the student's FAFSA information. For further instructions on how to obtain this form and information on other required forms, see How to Apply.
Master Promissory Note (MPN) for Federal Direct Loans
You should complete this promissory note after you have been awarded a Federal Direct Loan. This is not an application for a loan.
Master Promissory Note (MPN) for Federal Perkins, Nursing, and Health Professions Loans
You will only be able to complete this promissory note after you have been awarded a Federal Perkins Loan, Federal Nursing Loan, or Federal Health Professions Loan. This is not an application for a loan.
Non-Degree Student Status Form
For non-degree students who are required to take pre-requisite courses to be considered for admission into a degree program
PLUS (Parent) Loan Request for 2008-2009
For parents of dependent students who would like to request the PLUS Loan for the 2008-2009 academic year. Complete this form after receiving the 2008-2009 award notification from the Office of Student Financial Aid.
Primary Care Student Loan Statement of Rights and Responsibilities
For students who have been awarded a Primary Care Loan
RAP - Graduate Reasonable Academic Progress Appeal Form
Allows students to explain circumstances that prevented them from meeting RAP requirements
RAP - Undergraduate Reasonable Academic Progress Appeal Form
This appeal is for undergraduate students who have not met the minimum semester hour and/or grade-point average requirements of the RAP Standards.
RAP - Undergraduate Reasonable Academic Progress Duration of Eligibility Appeal Form
This appeal is for undergraduate students who have met or exceeded the duration of eligibility limits of the RAP Standards or are returning to earn a second undergraduate degree or teacher certification.
Release of Information Form
For students to provide authorization for the Office of Student Financial Aid to speak or correspond with another person/agency about their financial aid.
