If you would like to donate vacation time to any of these people, please fill out the appropriate form (Individual or Family) and send to: The Benefits Office, 120-40 USB, ATTN: Catastrophic Leave Donations.
Leave is not taken from your account until the employee actually needs it. It could be several months from the time we receive your donation form until the leave is used.
