Dental I and II Plans
Faculty and Professional & Scientific Employees
(Effective January 1, 2011)
You have two plans from which to choose: Dental Plans I and II.
Dental plans I and II both cover the same dental services such as routine exams, orthodontic care, and restorative care (such as crowns, fillings and root canals). They differ in the coinsurance amounts you pay, and in the way the maximum coverage is calculated.
How an Individual Uses Dental Services
Dental care is provided through Delta Dental of Iowa. Dental care may be obtained from almost any provider you wish. However, there are advantages to using participating providers who have contracts with Delta Dental of Iowa. A list of plan providers may be accessed via the web at: http://www.deltadentalia.com
Participating providers will accept payment arrangements and file claims for you with Delta Dental of Iowa. Payment is made directly to these providers.
Non-participating providers do not have contracts with Delta Dental of Iowa. They do not agree to accept payment arrangements and are not responsible for filing claims for you. Non-participating providers may charge more for dental care than participating providers. Payment is made to you and you are responsible for paying the provider.
How Much an Individual Pays for Dental Care Services
Although the two dental plans cover the same dental procedures, they differ in the coinsurance amounts you pay and in the way the maximum coverage is calculated.
| Plan | Benefit Period Cost |
|---|---|
| Dental I | No maximum annual benefit for Part A |
| Dental II | No maximum annual benefit for Part A $1500, per person, combined benefit period maximum for parts B and C |
The benefit period is the same as a calendar year. It begins the day your coverage goes into effect and starts over each January 1. This is true for as long as you have coverage.
