Instructions and Information for
Completing Your Personal Enrollment
These instructions will assist you in completing your Personal Enrollment for The University of Iowa Benefits Program.
All monthly costs are based on 12 premium payments per year. If you receive 10 paychecks per year, double deductions and credits will appear on your June 1 and September 1 checks.
All monthly premiums are paid using pre-tax dollars except for Spouse/Dependent Life Insurance, which is paid only on an after-tax basis.
NOTE: If any of the "Option Code" lines are blank, no coverage will be assigned to you for that program, except mandatory life and disability programs.
The definition of “spouse,” shall mean the person who is legally married to the employee under applicable state law, including a common law marriage, or a person who meets The University of Iowa definition of Domestic Partner, which requires an “Affidavit of Domestic Partnership” to be filed with the Benefits Office.
You may make a change to your benefits during the year only if there is a qualifying event.
Coverage begins the first of the month following the event (except for birth or adoption).
If you wish to make a change due to a qualifying event, a Benefit Change Request form must be submitted to the Benefits Office within 30 days of the event (60 days due to birth/adoption or loss if Medicaid or SCHIP coverage). The Benefit Change Request can be done online on the HR Self-Service web site, or a hardcopy of the Benefit Change Request form (pdf) can be found on the Benefits form site.
