Spring Semester Finals Child Care for Student Families
A collaboration between Little Angels Learning Center and the Family Services Office
Purpose –
To provide to student parents free, safe and quality child care during mid-term and final exams to achieve academic success. Little Angels Learning Center celebrates the uniqueness and acceptance of all cultural and diverse backgrounds.

Eligibility Criteria –
The student parent has a child under age 13 years and the student is
- currently enrolled at least half-time or is involved in academic responsibilities equivalent to half-time at the University (exceptions may be possible);
- is seeking a UI degree: undergraduate or graduate/professional/Master’s/Ph.D., and resides with the child when the care is needed.
Child Care Services Prior to Final Exams:
Dates and Times
- Saturdays, May 4 and May 11, 2013, 10 a.m. - 5 p.m.
- Sundasy, May 5 and 12, 2013, 1 p.m. - 5 p.m.
Student parents are responsible for --
- Completing and submitting all forms listed below.
- Providing infant cereal and/or baby jar food and prepared formula/breast milk in bottles.
- Canceling a reserved slot at a minimum of three days prior to scheduled services. Please note - If you do not cancel your reservation at a minimum of three days prior to scheduled services you could be denied from enrolling in future mid-term/finals child care programs.

All forms must be submitted when requesting the reserved child care. Child care is provided on a first-come, first-serve basis. Please print and complete all forms below (pdf files).

- Physical Examination Form - You may provide your own record or use this form
- Record of Immunizations - You may provide your own record, but it must be on the IA Department of Public Health Certification of Immunization form
- Permission Form - Photo/Field Trip
Care can only be provided on requested dates if completed forms are submitted.
