Initial Case Consult
HR Reps, Supervisors and Employees are encouraged to use this form for assistance with new questions, to initiate a new case or to update significant changes to an existing case that has not had recent involvement with FSDS. FSDS uses new consult information to appropriately assign a FSDS staff member. This staff member will respond to the named contact on the form.
This is a new support service and we encourage you to share your thoughts and suggestions.

Faculty and Staff Disability Services - UI Resources for Disability and Leave
The Faculty & Staff Disability Services (FSDS) office is charged with providing leadership, expertise and consultative intervention to all University of Iowa faculty, staff and department management in matters related to employment and health conditions. FSDS works towards the development and implementation of a University infrastructure capacity for people with disabilities to feel welcomed and supported; providing an employment environment of which other academic institutions aspire and that includes but is not limited to:
- A leave and disability system design that builds and sustains employee/employer relationships;
- A strong coalition of existing and new leave and disability resources that is visible and available to applicants, employees and employers;
- A centralized leave and disability service structure and coordination system for applicants, employees and employers; and
- A knowledge sharing system concerning disability within employment that reaches all employees.
Veterans
The University of Iowa welcomes veteran applicants to UI employment and works diligently to assure employees who are also veterans have opportunity for success. Some veterans have disabilities and whether these conditions may be attributable to military service or other reasons, many services are available to support veterans who are applicants, staff or faculty. Faculty and Staff Disability Services is available to answer questions, assist in helping with employment needs related to disability and make referrals to needed services.
