Pay: Leave is paid. Employee leave is considered paid work time and not charged to accruals.
Application: Absence is to donate bone marrow and/or organs (lung, liver, pancreas, kidney, intestine or other organ “that require the continuous circulation of blood to remain useful for purposes of transplantation)”.
Maximum Amount: Up to five (5) working days for a bone marrow donor; up to 30 days for organ donor. Time absent recorded on time record with appropriate codes and not charged to accruals. Leave is intermittent, reduced schedule, or continuous.
Eligibility: No length of service or leave duration requirements.
Authorization and Documentation: Department authorizes.
Situation: Employee has donated an organ.
Action and/or Documentation:
Department should request release to work when employee is able to return.
Employee has been absent two or more consecutive weeks.
Authorization Documents: Release to Work Letter and Form or similar document.
Restricted Work: Evaluate restricted work needs if applicable to job requirements, job flexibility, and other potential work assignment for the specific restricted work period necessary. Supervisors work closely with Dept HR Unit Rep.
Between Department and Employee
Who communicates with employee? Supervisor, HR Unit Rep, or Nursing Staffing Clerk
What is communicated to employee? Duration of granted leave is charged to FMLA if employee is eligible. Refer to http://www.uiowa.edu/hr/administration/forms/organ_donate.pdf. Two weeks prior to end of leave request release to work and if leave must be extended.
What communication is needed from employee? Medical verification of donation provided by physician or hospital conducting procedure.
How frequently should communication occur? Department determines necessary timeline for adequate scheduling of leave and work.
With Treating Healthcare Provider (THP)
Who initiates communication with THP? Employee
What is communicated? Employee should be required to provide evidence of the donation and length of leave. If extension of leave is necessary, additional information of duration of leave is required.
How frequently should communication occur? When clarification of the donation is needed to confirm the following:
- type of donation,
- duration
- return to work date
- limitations
With Co-Workers of Absent Employee
Limited to informing coworkers affected by the absence to information that employee is absent for health reasons and sharing the anticipated date of return. May need to discuss work load coverage during absence.
