Fiscal Faculty, Professional & Scientific, and SEIU
General Information
This form is used by Faculty employed on fiscal year appointments, Professional and Scientific and SEIU staff members employed in departments who are reporting vacation and sick leave usage on a monthly basis.
For Departments reporting their Monthly Leave Records through the Self-Service Center, the time record should be submitted to Human Resources within ten working days after the month ends.
If it is necessary to submit a second Monthly Leave Record to correct information on the original, please send a "Corrected Copy" Monthly Leave Record. The "Corrected Copy" Monthly Leave Record is available on the Payroll Forms web site for Human Resources. Please select the Monthly Leave Record, and check the "corrected copy" box.
A Monthly Leave Record for each month of employment will be created online under "Time Reporting" in the Self-Service Center at or near the beginning of each month.
Every staff member must complete and submit the form to the Payroll Department even with no usage.
Completing the Monthly Leave Record
Enter only those hours of absence with pay which are properly chargeable to vacation or sick leave. If you have a question as to what is properly classified as vacation or sick leave, contact your departmental administrator.
Four entries are provided for each day to indicate the various types of sick leave or vacation. Each entry must be coded with the proper number from the listing of paid leave codes in order to properly categorize the hours entered.
All entries are made in hours or tenths of an hour using the following schedule:
06 minutes = .1
12 minutes = .2
18 minutes = .3
24 minutes = .4
30 minutes = .5
36 minutes = .6
42 minutes = .7
48 minutes = .8
54 minutes = .9
If the time worked is not exactly even in tenths of an hour, then "round" to the next tenth of an hour.
Provision is also made for the election to have sick leave transferred to vacation. If the "every month possible" box is checked, the transfer will be made every month you are eligible until the election is cancelled.
