liveWELL helps you identify key health indicators that may lead to more serious health conditions and works with you to create reasonable, attainable goals for personal health improvement.

It's as easy as A-B-C!

 

Complete the Health Risk Assessment (accessed through Employee Self-Service) to help identify any specific health issues you may face now or in the future. Receive $50 for completing the HRA for the first time. Receive $65 for completing the HRA the subsequent year. A monthly drawing for $500 will include all who take their HRA in 2008!

 

Review your individualized report and use it to work on your personal health habits or share with your primary care provider.

 

Depending on the results of your Health Risk Assessment, you may qualify to work with a Health Coach; if you give consent to be contacted, a Health Coach can help you work towards your personal goals for maintaining a healthy lifestyle. If you are not contacted for this service, you can continue to participate in UI Wellness activities to keep yourself at lower risk for future health conditions.

* The IRS considers any monetary incentive or gift card given to an employee as cash equivalents which should be included in the employee's taxable income regardless of the amount. The $50/$65 given to you for completing the Health Risk Assessment will be reported to the Payroll Office the month you complete the HRA and reflected in your next month's pay check.

** An employee may only earn the monetary incentive or $500 drawing one time per calendar year.
