Exam Services (ES)
Student Guidelines


In order to ensure a quality exam service at Student Disability Services (SDS), the following procedures are in place:

  • Students must meet with their SDS advisor at the beginning of each semester to fill out Student Academic Accommodation Request (SAAR) forms. Students can call to schedule an appointment with their SDS advisor at 319-335-1462.

  • The student's SDS advisor will indicate on the form the accommodations for which the student is eligible. It is crucial that students talk with their instructor about any additional accommodations that he or she allows for each exam and that those are written on the SAAR form.

  • The student will need to contact and meet with the instructor to review the SAAR forms. The student and instructor need to discuss the approved date and time for taking exams as well as exam delivery and return. The SAAR forms require the student and instructor's signatures. The instructor keeps the pink copy and student keeps the yellow copy. The student must return the white copy to SDS.

  • SAAR forms are to be returned to SDS no later than the Wednesday of the week prior to the first test, for non-final exams, tests and quizzes, and the Wednesday three weeks before the test, for final exams.

  • SDS cannot guarantee the exam accommodation if return deadline is not met. If there is no room, the student will need to either take the exam with the class or contact the instructor to provide the accommodation.

  • We will adhere to the schedule written on the SAAR form, unless we receive approval from your instructor. It is important, therefore, that you arrive on time so that you have the full amount of time to complete the exam. If the instructor has specified the start time and you are late, you will not be given additional time; also you may not begin more than 10 minutes early.

  • If you are unable to take the test as scheduled, you need to notify SDS as soon as possible. However, it is your responsibility to contact your instructor to see if the test can be rescheduled.

  • It is your responsibility to remember your exam schedule. ES sends reminders only for your final exam schedule.

  • Before taking an exam at SDS, you will need to place bags and all materials not approved for the exam in an area designated by SDS.

  • For use with the exam, you will be allowed only the materials designated on either the SAAR form, the exam cover sheet or with notice from your instructor. Your instructor will be notified if you use other materials.

  • Once you begin taking an exam, you may leave the testing room only for quick restroom breaks or if breaks are allowed as part of your accommodations.

  • If you drop a course, you need to notify SDS as soon as possible.

  • The University of Iowa has a zero tolerance policy on cheating. If you are caught cheating, your instructor will be immediately notified.
