For a high clearance drug

 

CLH ~ QH

 

Since this drug displays negative inotropic effects, as you increase the dose, the cardiac output (CO) decreases. A decrease in CO will result in a decrease in QH. Since this is a high clearance drug, a decrease in QH will result in a decrease in the CL of the drug.
