How to get Recognized
- Complete the suggestion form on-line at http://www.bo.uiowa.edu/~fushr/cqi/SMART.cfm. You can also get a form by contacting the UI SMART Suggestion Program, 121-50 USB, or your HR Unit Representative. After completing the suggestion form, submit it to Pat Kenner, UI SMART Program, 121-50 USB.
