The Form A (Recruitment Plan) has been replaced with a new form the combines the old Requisition Form from Human Resources and the Form A from the Office of Equal Opportunity and Diversity.  The new P&S/Faculty Requisition/Recruitment Plan Form is available via the Human Resources Self-Service Website (in the HR Transaction System).
