If you have multiple forms of your test, you must submit an EES Scramble Sheet with your scoring request.  A copy of the scramble sheet is available at Scramble Sheet (print and fill out form) or Scramble Sheet (type in then print).  Also from the Forms link on the EES website.  You will be asked to indicate the location of each item on each form using item numbers.  An explanation is provided at the top of the scramble sheet.
