Injury Reporting

Work-related injuries/illnesses must be reported immediately to the employee's supervisor of department designee (University Operations Manual Chapter 34.2). The department designee must complete the First Report of Injury Form on-line within 24 hours after being notified of the incident.

Workers' Compensation

Questions regarding the Workers' Compensation form or claim handling procedures should be referred to the University Benefits Office. Supervisors should investigate the cause of incidents to prevent reoccurrence and to ensure that corrective action is taken. The Health Protection Office can be contacted by the department for technical assistance.

 HPO receives a copy of all Workers' Compensation claims. Incidents are summarized and categorized according to accident type, injury type, and body part. Departments can receive a copy of the claim activity for their area by contacting John Anderson at 335-9549.
