Application Instructions for the Undergraduate Major

To submit an application for the major in integrative physiology, please follow the steps below.

1. Fill out the IP Application form (Click here for the Application Form).

2. Print out a hard copy of your completed IP Application.

3. Attach an up-to-date ISIS transcript and/or any other formal transcripts to the IP Application.

4. You may deliver the IP application to the Departmental Office, 424 Field House, or mail your materials via US Postal Service to the address below:

If you are currently in school, please DO NOT submit an application during the fall semester while it is in session. Wait until the semester is over so that your transcript is up-to-date and includes the current fall semester.

Applications are due in the Departmental Office by February 28 for admission commencing the following fall semester.
