Listed below are some of the forms and applications used by The University of Iowa Office of Student Financial Aid. Please check that you are selecting forms for the appropriate summer or academic year period.

Applying for Financial Aid
Free Application for Federal Student Aid (FAFSA)--This form must be completed by U.S. students who are applying for financial aid awarded by the Office of Student Financial Aid.
Iowa Verification Form (IVF)--The IVF is required to complete your financial aid application. It is available on ISIS after The University of Iowa receives the information from your Free Application for Federal Student Aid (FAFSA). The 2012-2013 IVF will be available on ISIS mid-February. For further instructions on how to obtain this form and information on other required forms, go to our Application Process web page.
Summer Financial Aid Application--This form must be completed by students applying for financial aid for the summer session and will be available April 16, 2012. Students must be registered for their summer classes before completing this form.
Winter Financial Aid Application--This form must be completed by students applying for financial aid for the winter session. Students must be registered for their winter class before completing this form.
Loans
Master Promissory Note (MPN) for Federal Direct Loans--Complete this promissory note after you have been awarded a Federal Direct Loan. This is not an application for a loan.
Master Promissory Note (MPN) for Federal Perkins, Nursing, and Health Professions Loans--Complete this promissory note after you have been awarded a Federal Perkins, Nursing, or Health Professions Loan. This is not an application for a loan.
PLUS (Parent) Loan Request for the 2011-2012 academic year--Parents of dependent students must complete this request form if they want to accept the PLUS loan that was offered on their student's 2011-2012 award notification letter.
PLUS (Parent) Loan Request for the 2011 Winter Session--Parents of dependent students must complete this request form if they want to accept the PLUS loan that was offered on their student's award notification letter for the 2011 Winter Session
Cost of Attendance Adjustments
Use these PDF forms to request an increase in your cost of attendance if additional expenses are incurred that could affect your or your family's ability to contribute to your college education. For detailed information, go to our Special Circumstances web page.

2011-2012
Student's Childcare Expenses--Complete this form if you (the student) have childcare expenses.
Parent's Childcare Expenses--Complete this form if you are a dependent student and your parent has childcare expenses.
Computer Purchase--This policy allows for one computer purchase as an undergraduate and/or one computer purchase as a graduate or professional student. If you are an entering freshman, transfer student, or first-year graduate student, you must purchase your computer after June 1, 2011.
Medical/Dental/Nursing Home Expenses Incurred by Self-Supporting Students --Complete this form if you are a self-supporting student and have extensive medical/dental expenses.
Medical/Dental/Nursing Home Expenses Incurred by Parents of Dependent Students--Complete this form if you are a dependent student and your parents have extensive medical/dental expenses.
Transportation Expenses--Complete this form if you have extensive transportation expenses.
Satisfactory Academic Progress (SAP)
Satisfactory Academic Progress Appeal Form--This form allows students to explain circumstances that prevented them from meeting SAP requirements.
Miscellaneous
Release of Information--This form is for students to provide authorization for the UI Office of Student Financial Aid to speak or correspond with another person/agency about their financial aid.
Non-Degree Student Status Form--This form is for non-degree (A9, AE, G9, G3, etc) students who are required to take pre-requisite courses to be considered for admission into a degree program.
Employers of Off-Campus Work-Study Students

Off-Campus Work-Study Appointment Request--Use this form to start the hiring process for a work-study employee.
Rate of Pay Increase--Use this form to increase the hourly rate of pay for a work-study employee.
Termination Request--Use this form to terminate the employment of a work-study employee.
