Send a file + Prepress order form + Standard UI requisition form + MFK addendum form

Use this checklist to make sure you send all the information we need to complete your printing order. We have compiled some tips to help your order move smoothly through our plant. Feel free to contact one of our customer service representatives if you have any questions.

1. Get a requisition.

2. On it, write:
____Your MFK number - your departmental office staff will have this.
____Whether the job is new, a reprint, or a revision.
____Last job number, if the job is a reprint or revision.
  ____Quantity you want.
____Signatures. Your department's authorization signatures are required.
____Brief description. For example, single-fold brochure on 11x17 white cover stock; 3-part NCR form.
____Whether you want to see a proof.
____When you want the job delivered. Be specific: ASAP and RUSH mean different things to different people.<
____Where you want the job delivered. Central Mail, your office, a receiving dock? Specify person, room, and building.
____Sample or mock-up - especially important if job prints two sides or folds.
  ____CD or disk, if applicable.

3. Send or bring everything on this checklist to:
Printing Department
100 MBSB
2222 Old Hwy 218 South
Phone 384-3700, fax 384-3707

Very important: If you are sending a file over the web, you must still send a requisition. We cannot begin any work on your order until we receive it!
