Student Academic Accommodation Request
(SAAR) Forms

 

The SAAR form is a document generated by SDS that ensures a student has a documented disability and has been deemed eligible for specific academic accommodations. The information on this form is confidential and should be treated as such.

Please do not provide accommodations to students without this form.

As an instructor, you are asked to provide or arrange the accommodations for a student in your class who presents a SAAR form to you. The procedure for processing these request(s) is as follows:

  1. Confirm that the instructor contact information is complete and correct.
  2. Review the SAAR with the student and discuss the specifics of each accommodation request
  3. If SDS will provide exam accommodations, complete the back of the form.
    • Select exam delivery and return methods
    • List all exam dates with start time, time allowed for class, items allowed and a contact number during exam
  4. If you would like a copy of the SAAR form, please indicate below signature line on the form.

 

Exam Accommodations

Instructors should provide testing accommodations as they are able. Instructors have the option of using SDS Exam Services to provide testing accommodations. Go to Exam Services Guidelines for more information.
