Description:
Lantern Park is a non-profit organization that offers skilled and intermediate care. The purpose of the Medical Social Services Department is to provide services to residents and families to promote optimal social functioning and to enable residents to reach their highest practical level of functioning. Services include: being a member of the interdisciplinary care plan team, participating in Quality Assurance meetings, resident advocate, providing information and referrals, psychosocial assessment, discharge planning, admission screening and determining Medicare skilled criteria including completing Medicare documentation/denials.

Description:
Provide staff support to the Linn County Decategorization Project Executive Committee by researching and preparing appropriate planning materials. Participate in the development of the Decategorization Plan by formulating goals/objectives, time lines, performance indicators, etc. Staff Advisory Committee meetings and record activities.
Background:
Linn County has been approved by the Iowa Department of Human Services as a Decategorization Project. The goal of this project is to create a single child welfare fund from multiple separate funding sources to better serve client and community needs.
