Description:
Mental Health and Chemical Dependency Associates (MH & CD) provides psychiatric and substance abuse evaluation and access to ongoing care. Evaluation begins with Triage. Triage is the process of assessment and selection of care provided by a licensed mental health professional. Triage is always the first step in accessing a client’s MH and CD benefits. A careful social and clinical history is taken to define the issues and select the provider of choice, along with any special recommendations.
Also, a division of our company is Cedar Valley Employee Assistance Program. This service offers brief therapy for clients prepaid by the employer.

Description:
Mental Health Clinic of Tama County provides rural mental health outpatient services that include: individual, marital, family, and group therapy using a systems orientation based upon Bowen/structural strategies. In addition, community support service and group support services for chronically mental illness, elderly outreach counseling and critical incident debriefing services are provided.

Description:
As a teaching hospital, with students from approximately 15 different disciplines we are used to having students. Throughout the hospital we work in teams and students are treated as professionals.
Programs: (1) The Cromwell Children’s Unit serves children at the grade school level. Patients and their parents are provided a full range of casework services. These children come from all over the eastern half of Iowa. Two social workers and students are responsible for therapy to the parents as well as coordination of the team and of services with community agency personnel who are involved. Average length of stay is 3 months. All children are in school programs. (2) The Adolescent Unit serves youths at high school level for a full range of mental illnesses and behavior disorders. The youths also come from all of the eastern half of the state. The social worker is responsible for the initial social work assessment, development of the treatment plan and certain administrative responsibilities. On a limited basis on-going casework services (full range) and placement are also required. If the primary problem is substance abuse the patient will be referred to another facility for that specific treatment. Average length of stay is 2 months. (3) Adult Psychiatric Services for men and women provide opportunities for students to be trained in a full range of casework services. The social worker is responsible for intake assessment, casework, and placement services. We coordinate with patients, families and community organizations. Average length of stay is 28 days.

Description:
The staff provide social and psychological support to patients, family members, and employees. Services provided to in and out-patients, and include individual and group work. Work in conjunction with the total health care team to provide holistic approach. Services include: 1) Hospice: counseling
or terminally ill patients and their families; 2) Discharge Planning: for elderly and disabled regarding setting up placement; 3) Long Term Care: involvement with elderly at residential level of care; 4) Renal Dialysis Unit: out-patient unit for individuals with chronic renal failure; 5) Psychiatric Unit: individual and group work for in-patients with emotional or mental problems; 6) Support Groups: for cancer, heart, diabetes, bereavement.

Description:
Mercy Medical Center offers a full range of acute care services including basic medical and surgical services in addition to cardiac services, chronic obstructive pulmonary disease services and physical rehabilitation services.
Behavioral services include mental health and substance abuse services provided for both inpatients and outpatients. Hospital-based skilled nursing units are at both of Mercy Medical Center’s locations--Dyersville and Dubuque. Mercy also operates a 40-bed ICF facility in Dyersville. Home health services through Mercy are available to clients as needed.
Student placements have been structured to provide a broad overview of social work activity in health care along with a concentrated experience in an area of a student’s preference.

Mercy Behavioral Services offers a field placement for students in their second year of the MSW program. Our goal of field placement is to build upon the generalist preparation of the foundation year. We also strive to provide an opportunity for the social work student to deliver social work services to individuals, families, and groups with consideration of the contexts of their social environments and at a level expected of advanced professional practitioners. Description:
The Mercy Employee Assistance Program is designed to assist employees and their families with problems that are affecting their job performance and their personal lives. Problems such as alcoholism and drug abuse, divorce or death of a spouse or family member, family problems, marital discord, emotional problems, financial difficulties and legal concerns. Staff meets to jointly assess and identify the nature of problems and provide counseling or assistance necessary.

Description:
Meth-Wick Community is Iowa's only accredited Continuing Care Retirement Community (CCRC). Our 58-acre campus serves approximately 370 retired persons through independent living, assisted living, or nursing home care. Independent residents live in apartments, condominiums, or duplexes. Our assisted living area provides assistance with ADLs for those with physical impairments. Arbor Place is our 32-bed early dementia facility for those with cognitive impairments, such as Alzheimer's Disease and Vascular Dementia. Our 65-bed Intermediate Care Facility was constructed in 2000 and features a 24-bed CCDI Unit. We also offer respite care services, and will soon provide hospice care.
Students may choose to work in any area(s) on campus. Students may choose to practice medical crisis counseling and adjustment to illness.

Description:
Mid-America Housing Partnership (MAHP) is a private, non-profit affordable housing development and property management organization. Currently MAHP manages over 450 rental units, including housing for the mentally challenged and units for senior citizen independent living. In addition to this, the organization constructs new single family homes in Cedar Rapids area and in rural communities throughout the state. MAHP also rehabilitates older homes to preserve housing stock, historical and otherwise, in Cedar Rapids. Educational opportunities for potential home buyers, tenants, and landlords are also available through MAHP. In addition to creating affordable housing programs, MAHP also has a job-training program for at-risk youth (Eco Youth), and an employment program for teens who are interested in learning how to design and maintain web sites (Spider Web Designs). Unique Learning Opportunities for Students:
All interns and practicum students are provided training in consensus decision making and meeting facilitation, including opportunities to utilize the knowledge they gain in internal meetings at MAHP. MAHP also provides interdepartmental training and activities, so all students placed have an opportunity to learn about each department in the organization. A full time Community Facilitator is also employed, to work with the residents of MAHP properties and link them to the services and resources that will help them to remain stable. By working with the Community Facilitator and other members of the MAHP staff, students are able to learn about the programs available in the community, and interact with working families and individuals to better understand the problems they face. MAHP's Board of Directors, policy councils, and resident councils, all comprised of volunteers from the community, are able to provide a valuable link to the businesses and organizations which play an active part in the growth and economic stability of the Cedar Rapids area. Every employee of MAHP is dedicated to sharing their knowledge with the students who are placed at the organization, ensuring their growth and success. Staff have spent a great deal of time developing a strong internship and practicum program at MAHP, which is evident in the fact that approximately 20% of the 50 students that have been at MAHP in the past eight years have found long-term employment at the organization.
Expectations:
MAHP seeks employees, volunteers, interns, and practicum students who are positive, willing to learn, responsible, and ready to try new things. A willingness to make a difference in the community and a sense of humor are also important.

Description:
Agency’s purpose is to provide community-based assessment, treatment, and educational services for substance abuse and related problems regardless of ability to pay for services. Intake and assessments are provided for all persons seeking services at MECCA. The student will interview and formulate appraisal of potential substance abuse treatment needs, and participate in referral coordination and case management. The Outpatient-Inpatient Substance Abuse Treatment is for current MECCA residents, and the student will participate in individual, group, and family counseling; treatment plan assessment and formulation; and related case management.

Provides assessment, short term counseling and referral services to employees and family members of companies that have contracted with the Employee Assistance Program. Also participate in organizational consultation regarding employee performance problems.

Description:
Purpose of program: The FSS program engages families (where a parent has been identified as having a persistent mental illness) in a process of self-assessment, strength identification, family goal-setting, and establishment of formal and informal supports.
Role of Practicum Students: Assisting the program in collecting data for a program evaluation. The program evaluation (still in the process of coming together) will most likely include the following tasks: 1) Obtaining medical releases from FSS clients to quantify number and duration of psychiatric hospitalizations over lifetime; 2) Obtain and quantify number and duration of foster care placements of FSS client's children; 3) Administer "Quality of Life" instrument to FSS clients. The student will also have the opportunity to shadow FSS workers on daily tasks with clients to learn more about the program's purpose and process.
Scope of the project: The evaluation will be limited to 26 open FSS families.
Type of student: The student should have an interest in working with mental illness or research.

Description:
Milestones Adult Day Health Center serves physically and psychologically challenged adults over the age of 18, with the primary mission being to keep participants independent and in the community. Milestones serves approximately 40 individuals daily, with space available for 60. A special care area for 16 individuals experiencing dementia is available. Milestones is a not for profit agency, a program of Aging Services, Inc., a subsidiary of Abbe Inc. Milestones is designed as a national demonstration site in changing the paradigm of long term care and adult day health services via holistic healing methodologies. Clinical experiences are available to undergraduate students. Both clinical and administrative experiences are available to graduate students. Milestones is an ideal setting for students wishing experiences with older adults, primary caregivers, and gerontological community support systems or administrative experience in long term care.

Placements involve working with children with special education needs and their families. Activities may include group and individual counseling work, education plan development, and family work as part of an interdisciplinary team. Placements in West Liberty and Columbus Junction offer opportunities to do cross-cultural social work with Spanish speaking and some Southeast Asian families.
